1. PROVIDER OF DIGITAL SERVICES
  1. The Terms and Conditions shall apply to digital services provided by Iwona Gnach-Olejniczak who runs a business under the name: UNIDENT UNION I&II Iwona Gnach-Olejniczak at the following address: ul. Kochanowskiego 20 lokal CD, 51-601 Wrocław, NIP: 8991066730, REGON: 890537707 (“UNIDENT UNION”) and her employees and associates.
  1. TYPES AND EXTENT OF DIGITAL SERVICES
  1. UNIDENT UNION provides digital health services related to diseases of the teeth, mouth, facial part of the skull and adjacent areas. The Services consist of an initial analysis of the problem reported by the Customer and the images attached by the Customer and the preparation of a detailed treatment plan (“Services”).
  2. The digital services provided by UNIDENT UNION are only an analysis of the problem reported by the Customer. To start treatment, the Customer must visit the clinic run by UNIDENT UNION for detailed dental examination.

III. CONDITIONS FOR THE PROVISION OF DIGITAL SERVICES

  1. The services are provided only to persons who fulfill the following conditions (“Customers”):
  • are aged 16 years and over,
  • have read the information available on www.unidentunion.pl in the “GDPR” tab,
  • have agreed to the processing of their personal data in order to answer the question asked during the online consultation,
  • have correctly completed and submitted the contact form via www.unidentunion.pl (the “ONLINE CONSULTATION” tab).
  1. To use the IT system intended to provide the Services, you need access to a device with Internet connection and to an e-mail box.
  2. It is prohibited to provide illegal content through the IT system intended to provide the Services, in particular content that could infringe the rights of third parties (e.g., by sending data of third parties without their consent).
  1. AGREEMENT FOR THE PROVISION OF DIGITAL SERVICES
  1. When UNIDENT UNION receives a properly completed contact form submitted by the Customer via www.unidentunion.pl (the “ONLINE CONSULTATION” tab), an agreement is concluded between UNIDENT UNION and the Customer for digital services in accordance with these Terms and Conditions (the “Agreement”).
  2. The Services are provided free of charge.
  3. The Agreement shall be terminated:
  • within 7 days of the date on which UNIDENT UNION has received a properly completed contact form submitted by the Customer (the termination of the Agreement does not require any additional statements from the Customer) or
  • upon receipt of a statement from the Customer that they do not wish to receive the Services, either sent electronically or otherwise submitted in a document form.
  1. UNIDENT UNION is not responsible for:
  • false, incorrect or incomplete information provided by the Customer,
  • errors in data transmission, in particular due to failure of the IT systems, telecommunications systems, power supply, and for reasons attributable to the external data transmission operator,
  • no access to the IT system intended to provide the Services for reasons beyond the control of UNIDENT UNION
  1. COMPLAINTS
  1. The Customer has the right to submit complaints regarding the Services or the Agreement.
  2. Complaints may be submitted in:
  1. The complaint should contain the following information:
  • the details of the person submitting the complaint (first name and surname) and contact details: correspondence address or e-mail address,
  • description of the Service or Agreement to which the complaint pertains,
  • description of the problem that has caused the complaint,
  • a request from the Customer regarding the expected complaint resolution,
  • the information on how the Customer wishes to receive a response to the complaint,
  • signature – in the event of the submission of the complaint in writing.
  1. Complaints that do not allow for the identification of the Customer shall not be processed.
  2. A response to the complaint shall be provided within 30 days of the date when UNIDENT UNION receives the complaint.
  3. In the event that additional information is required to process the complaint, UNIDENT UNION may request it from the Customer. Failure to provide the necessary information by the Customer may result in rejection of the complaint.
  1. PERSONAL DATA PROCESSING
  1. To provide the Services and to consider claims, UNIDENT UNION processes the personal data provided by the Customer.
  2. The Personal Data Controller is Iwona Gnach-Olejniczak who runs a business under the name: UNIDENT UNION I&II Iwona Gnach-Olejniczak at the following address: ul. Kochanowskiego 20 lokal CD, 51-601 Wrocław, NIP: 8991066730, REGON: 890537707.
  3. Other information regarding the processing of personal data can be found on www.unidentunion.pl in the “GDPR” tab.

VII. FINAL PROVISIONS

  1. The Terms and Conditions shall enter into force on [January 1, 2019].
  2. UNIDENT UNION has the right to amend the Terms and Conditions at any time.
  3. The amended Terms and Conditions will be published on www.unidentunion.pl. Notwithstanding the foregoing, UNIDENT UNION shall notify the customers who concluded an Agreement with UNIDENT UNION whenever the Terms and Conditions are amended.
  4. The new Terms and Conditions shall be binding on new Customers and existing Customers who do not cancel the Services within 14 days of the date of notification of the amendment to the Terms and Conditions.
